Mobile Care Group arranges for "on-site" healthcare services in Nursing Homes, and Assisted & Independent Living Facilities. Specifically, we coordinate the following: Optometry, Audiology, Podiatry, Psychiatry, Psychology, and Dentistry.
* We recruit local physicians who are committed to providing excellent patient care. Because our physicians are local to your facility, they are able to offer urgent care services. * We offer "Single Point of Contact" capabilities for all services. You'll have one contact person to address all of your concerns.
* We provide
consistent on-going patient care. We provide regular scheduled visits - typically monthly visits - based on your needs. This consistent schedule prevents conflicts with other facility activities.
* We collect patient information, obtain medical authorizations, contact the patient's family when necessary, ensure insurance eligibility, and complete all billing. We do it all.
* We provide the administrative support that often rests on the responsibility of the facility Social Worker. We are keenly aware of the workload on the social worker and try to off-load as much of that work as possible.
Clinical Expertise [back to top] MCG's affiliated network of clinical staff is committed to enhancing the quality of life by providing the best care possible for each patient. Clinically, MCG-affiliated doctors review the patient’s complete medical history to determine the best action plan. They work closely with attending physicians and ancillary staff of the facilities, charting all pertinent information,
including medications. They complete an extensive examination utilizing the most technological advanced mobile examination equipment.
MCG is affiliated with national laboratories. During the monthly on-site visit, physicians determine if new aids (glasses, hearing aids, dentures, and diabetic shoes, etc.) are needed. If entitled and/or if approved by the family or POA, aids will be ordered. They are then engraved with the patient’s initials and dispensed and adjusted during subsequent visits.
MCG has an International Network of Physicians from which to choose. These doctors are local, therefore, MCG is able to offer urgent care services. The doctors are affiliated with International Schools and National Associations. MCG co-manages surgical procedures if needed. MCG is HIPPA compliant.
Consistent Patient Care [back to top] MCG works with the facility social worker to coordinate monthly on-site visits. We provide
consistent care to new patients and follow-up on patients with signs of systemic diseases. A typical monthly full-day visit consists of routine examinations on 10 to 12 patients and 4 to 6 follow-up examinations.
Dignified Care [back to top] MCG coordinates quality health care in a dignified manner to the growing number of nursing home and assisted living residents around the country. MCG affiliated doctors are committed to providing your residents with the quality care they are entitled to. For instance, we know that one concern the residents and their family have is the adjustment to a newly dispensed aid. Our affiliated doctors will take time to work through these issues and concerns with the patient.
Managerial Care [back to top] In many cases, human resources in senior care facilities are stretched to capacity. MCG partners with your facility to customize a unique care platform that works for your patients, their families and your staff. We enhance your productivity by handling everything from scheduling, coordination of benefits, billing, exams, aid dispensing to streamlining the surgical process when necessary. Our regional coordinators have years of industry experience and work closely with the social worker and Director of Nursing to best meet the facility’s needs and daily routine. Our processes are structured but flexible from facility to facility. We also provide “In-Service” training programs for nursing home staff.
Top Notch Services [back to top] IMPLEMENTATION IS SEAMLESS!!! After the nursing home administrator authorizes MCG to begin providing health care services, MCG staff will begin working with the social worker right away. MCG will select doctors from our National Network of qualified physicians and settle on the nursing home visit schedule. We collect all necessary patient information, coordinate medical authorizations and family consents, check on patient insurance eligibility, and complete all billing.
After each visit, MCG provides the social worker with a Patient Summary Form that concisely lists the patients seen and treatments recommended or received that day. MCG will customize the implementation for each facility and will do as much as the facility needs.
