On-site Healthcare Services for Senior Care and Assisted Living Facilities
Mobile Care Group coordinates and manages programs for audiology, dentistry, optometry, podiatry, and psychiatric/psychological services that provide on-site health care for nursing home and assisted-living residents. We are HIPAA compliant and approved by Medicare and Medicaid. Our services are covered by most health insurance policies, and we accept private pay.

We are your facility partner, and our goal is to provide the expert, on-site care patients deserve. We recruit local physicians who are committed to providing the highest level of care, and because they are local, they can meet patient needs for urgent-care services as well.

We work with and complement the responsibilities of the facility social worker by coordinating on-site visits and providing consistent patient care and follow up services. We will attend patient care and family meetings, if needed, and in all cases, our top priority is ensuring the care and dignity of our patients.

Audiology/Hearing Services

Hearing is critical to seniors’ emotional and physical health. Signs of hearing loss may include difficulty hearing the television or radio; having to ask that words be repeated; speaking too loudly; or disengaged in general conversation. The National Health Council on Aging recommends annual hearing evaluations for seniors to evaluate hearing loss. Hearing aids can be prescribed and ordered as necessary to improve hearing.

Dentistry/Oral Health Services
Recent studies conclude that poor oral health heightens risk factors for certain types of diabetes, cardiovascular and respiratory diseases, especially among elderly people. With more older people retaining their natural teeth for life, good oral care is becoming increasingly important. That’s why we recommend that all residents receive a dental exam and follow-up care to eliminate potential discomfort and maintain function and systemic health. Dentures may be ordered as necessary.

Because dental services are not covered by Medicare or most other insurance programs, Mobile Care Group has partnered with Senior Dental Insurance Solutions (SDIS) to introduce the Oral Cavity Rehabilitation Program (OCRP). This innovative program enables us to provide nursing facilities with dentists and hygienists specially trained to deliver a dental program that recognizes oral care as the foundation for maintaining overall good health.

Mobile Care Group provides:

• full oral examination and diagnosis & treatment of the entire oral cavity
• treatment of gum diseases, cleaning, extractions, fillings and appropriate follow-up care
• high-tech portable dental equipment
• a preventative and rehabilitation program that includes monthly oral cavity care
• urgent care services as needed

• a comprehensive order department that includes complete denture services – fitting & fabrication, labeling, dispensing, and all relines and adjustments

Optometry/Vision Services
Mobility and visual performance are essential for seniors to function fully. We recommend that all seniors receive an annual eye exam and appropriate follow-up care. We also work with facility staff members to ensure understanding of specific visual challenges of each patient. This information is then incorporated into an individualized rehabilitation plan.

Mobile Care Group provides:

• full eye examination and diagnosis and treatment of the eye
• treatment of eye diseases – cataracts, glaucoma, macular degeneration – and all appropriate follow-up care.
• high-tech portable optometric equipment
• a preventative eye care program that includes communication of visual challenges to facility staff and training to enhance therapy
• urgent care services as needed
• co-management of off-site surgical procedures when necessary
• a comprehensive order department that includes complete eyeglass services – identification of prescription changes, fitting and ordering of glasses, engraving frames, dispensing, and all adjustments as needed

Podiatry/Foot Care Services
Corns, calluses, diabetic related foot complications and other foot problems can make walking painful. Even simple tasks such as trimming of toenails can become difficult, if not impossible for seniors or disabled patients.

Mobile Care Group provides:

• full foot examination and diagnosis and treatment of the foot
• treatment of foot problems – vascular, corns, calluses, foot pain, debridement – trimming of toenails
• urgent care services as needed
• co-management of off-site surgical procedures when necessary
• referrals for diabetic shoes, inserts and fitting as necessary
• assistance with scheduling, eligibility, family contact, and billing

Psychiatric and Psychological Services
A sense of emotional wellbeing and support is important to all of us. At times, life situations and health issues can become difficult to manage, especially for seniors. With Mobile Care Group, help and support are readily available. Our Nurse Practitioners (NP), in collaboration with Psychiatrists, will access the patient and help determine a treatment program. Our Licensed Independent Social Workers (LISW) provide psychological care and support. All treatment options and recommendations will be coordinated with the patient’s primary care physician. As in all of the healthcare services we coordinate, patient confidentiality is ensured.

Mobile Care Group provides:

• NP’s who work with the Medical Director to manage all psychotropic medications according to current Omnibus Budget Reconciliation ACT (OBRA) guidelines. Medications are reviewed monthly or as often as necessary
• NP’s complete all required Abnormal Involuntary Movement Scale (AIMS) testing
• all documentation is made available to the facility on the day of the visit
• all NP visits count as physician visits; are scheduled regular and/or as needed
• LISW’s provide follow-up on all psychotherapy referrals made by the facility or NP
• presentation and facilitation of free in-service seminars for facility staff covering such topics as: behaviors in long-term care, managing residents with behavior issues, managing difficult family members, teamwork, preventing burnout, and other pertinent subjects
• attendance at family meetings and/or care conferences as needed to further support the facility staff and the resident
• assistance with scheduling, eligibility, family contact, and billing
