On-site Healthcare Services for Senior Care and Assisted Living Facilities
Mobile Care Group coordinates and manages programs for audiology, dentistry, optometry, podiatry, and psychiatric/psychological services that provide on-site health care for nursing home and assisted-living residents. We are HIPAA compliant and approved by Medicare and Medicaid. Our services are covered by most health insurance policies, and we accept private pay.
We are your facility partner, and our goal is to provide the expert, on-site care patients deserve. We recruit local physicians who are committed to providing the highest level of care, and because they are local, they can meet patient needs for urgent-care services as well.
We work with and complement the responsibilities of the facility social worker by coordinating on-site visits and providing consistent patient care and follow up services. We will attend patient care and family meetings, if needed, and in all cases, our top priority is ensuring the care and dignity of our patients.

Hearing is critical to seniors’ emotional and physical health. Signs of hearing loss may include difficulty hearing the television or radio; having to ask that words be repeated; speaking too loudly; or disengaged in general conversation. The National Health Council on Aging recommends annual hearing evaluations for seniors to evaluate hearing loss. Hearing aids can be prescribed and ordered as necessary to improve hearing.

Dentistry/Oral Health Services
Recent studies conclude that poor oral health heightens risk factors for certain types of diabetes, cardiovascular and respiratory diseases, especially among elderly people. With more older people retaining their natural teeth for life, good oral care is becoming increasingly important. That’s why we recommend that all residents receive a dental exam and follow-up care to eliminate potential discomfort and maintain function and systemic health. Dentures may be ordered as necessary.
Because dental services are not covered by Medicare or most other insurance programs, Mobile Care Group has partnered with Senior Dental Insurance Solutions (SDIS) to introduce the Oral Cavity Rehabilitation Program (OCRP). This innovative program enables us to provide nursing facilities with dentists and hygienists specially trained to deliver a dental program that recognizes oral care as the foundation for maintaining overall good health.

Optometry/Vision Services
Mobility and visual performance are essential for seniors to function fully. We recommend that all seniors receive an annual eye exam and appropriate follow-up care. We also work with facility staff members to ensure understanding of specific visual challenges of each patient. This information is then incorporated into an individualized rehabilitation plan.

Podiatry/Foot Care Services
Corns, calluses, diabetic related foot complications and other foot problems can make walking painful. Even simple tasks such as trimming of toenails can become difficult, if not impossible for seniors or disabled patients.

Psychiatric and Psychological Services
A sense of emotional wellbeing and support is important to all of us. At times, life situations and health issues can become difficult to manage, especially for seniors. With Mobile Care Group, help and support are readily available. Our Nurse Practitioners (NP), in collaboration with Psychiatrists, will access the patient and help determine a treatment program. Our Licensed Independent Social Workers (LISW) provide psychological care and support. All treatment options and recommendations will be coordinated with the patient’s primary care physician. As in all of the healthcare services we coordinate, patient confidentiality is ensured.
